Hyperhidrosis is excessive sweating that is not driven by temperature or exertion. In its focal form the sweat glands in one region — most often the underarms (axillae), palms or soles — fire far more than the body needs, soaking through clothing and making grip difficult. Botulinum toxin type A interrupts this at the nerve. The same protein used to relax expression muscles also blocks acetylcholine, the chemical messenger that tells the eccrine sweat glands to produce sweat, so the treated glands quieten down.
The doctor maps the active area first — for the underarms a simple starch-iodine test can show where sweating is heaviest — then places a grid of small injections just under the skin so the toxin reaches the gland layer. Because the effect is local, only the treated zone is affected, and the body still regulates its temperature through the rest of its skin. The number of injection points and units depends on the size of the area; the doctor sets these at the consultation rather than from a fixed package.
A doctor confirms the sweating is focal, reviews your medical history and any medication, and may run a starch-iodine test to map exactly where you sweat most.
The area is cleansed and marked. A topical numbing cream or cooling is used for sensitive zones such as the palms and soles; the underarms usually need little.
