Dissolving Lip Filler: When Hyaluronidase Is Used and What to Expect

The short answer

Doctors dissolve lip filler with hyaluronidase, an enzyme that breaks down hyaluronic acid (HA). It works on HA fillers only, which includes the lip fillers described on the clinic’s page, and it is used when filler has migrated, formed lumps, looks uneven or adds more volume than you want. Nobody can predict the result to the last drop: some of your own HA goes too, and a second visit is sometimes needed. Your doctor examines the lips first and sets the plan.

How hyaluronidase breaks down lip filler

Most lip fillers are cross-linked HA gels. Hyaluronidase cuts the HA into smaller fragments that the body then clears. Your body already makes similar enzymes: a 2025 review in Plastic and Reconstructive Surgery Global Open notes that two human enzymes, HYAL1 and HYAL2, work in sequence to break HA into smaller fragments that leave the body. A doctor injects a medicinal form of the enzyme straight into the filler instead of waiting for the slow natural process, which is why HA fillers can be dissolved while fillers made of other materials do not respond.

The enzyme is not licensed for this purpose. The US prescribing information for one recombinant hyaluronidase lists three uses: as an adjuvant in subcutaneous fluid administration, to increase the dispersion and absorption of other injected drugs, and in subcutaneous urography. Dissolving dermal filler is not on that list. The clinic’s filler removal treatment page describes it as an off-label use that is accepted practice in aesthetic medicine, and you consent to it in writing.

When doctors use it on lips

Lips move all day and swell easily, so problems with filler show quickly. A doctor may suggest dissolving in these situations:

  • Filler has spread beyond the lip border or shifted from where it was placed.
  • Lumps or an uneven texture remain after the swelling has gone.
  • The lips look overfilled, or the upper and lower lip no longer match.
  • You want to return to your baseline before planning a new treatment.
  • A complication such as a blocked blood vessel needs treatment at once.

Dissolving is not the first answer to every lump. The FDA summary for Restylane Kysse says that lumps and bumps after injection may last longer than the first week but typically resolve in less than 30 days. The same document lists enzymatic degradation with hyaluronidase among the treatments used for reported adverse events, next to corticosteroids, antibiotics and antihistamines. A doctor therefore looks at the timing, the texture and the symptoms before choosing between waiting, medication and dissolving. Our article on lip filler migration, lumps and complications explains what each sign means.

What dissolves and what does not

Only HA filler responds to hyaluronidase. Fillers made of other materials, including those that stimulate your own tissue to produce collagen, need a different medical approach, and the treatment page says so directly. This is why the first question at a consultation is which product you received, when and where. Bring the treatment record or the product sticker if you have one. If you do not know what was injected, tell the doctor, who will examine the area and ask about the history before deciding.

What the treatment involves and how fast it works

The treatment page describes a short visit: the doctor checks the filler history, applies numbing cream, and injects the enzyme with a fine needle. The injection itself takes minutes, and you may feel mild stinging. An allergy patch test may be advised first.

Timing depends on the gel. A 2025 review describes laboratory experiments in which the enzyme mixed with a looser biphasic filler within about 5 minutes and dissolved it almost completely within 1 to 2 hours. A tightly cross-linked monophasic filler took about an hour before the enzyme began to permeate it, and dissolving could run up to 24 hours. In practice, the treatment page says softening and visible reduction usually show within 24 to 48 hours, and the doctor judges the final result at about two weeks. The same review advises waiting at least a few days to a week before giving more enzyme, so the doctor can assess the first dose.

Risks and limits

Dissolving has four limits worth knowing before you agree to it.

  • Over-dissolving. The treatment page explains that the enzyme is not selective and also breaks down your own HA. The 2025 review warns that excessive use can dissolve more filler than intended, and the treatment page adds that the area can look flatter or thinner than before any filler was placed until your own HA is replaced over days to weeks.
  • Uneven results. How much dissolves cannot be predicted exactly. Dense or layered filler may need a second session.
  • Allergy. The US label lists known hypersensitivity to hyaluronidase as a contraindication. A scoping review in Aesthetic Plastic Surgery puts IgE-mediated type I hypersensitivity at about 0.1%, which is rare but real. Tell your doctor about any allergy to bee or wasp stings and any earlier reaction to the enzyme.
  • Limited evidence for the face. The same scoping review found five randomized trials with 53 subjects in total. They tested removal of HA nodules on the forearm, upper arm and back, and the authors found no randomized trials, clinical trials or case-control studies on removing HA from facial skin. With that gap in the evidence, a doctor’s experience and caution matter.

Swelling and bruising at the injection points are also possible, and the treatment page says swelling is usually most marked for two to three days.

Planning new filler afterward

Many people dissolve filler to start again, not to stay without it. The treatment page says the doctor usually advises a short gap, often a couple of weeks, so swelling settles and the tissue calms before new gel goes in. Your doctor sets the exact schedule for your treatment. If you later plan a new lip shape, the lip filler treatment at the clinic page explains how the doctor plans the amount and the technique, and our article on how the amount of lip filler is decided covers the reasoning behind a cautious start.

If you are weighing dissolving against waiting, a consultation lets a doctor examine the lips and give advice for your case: arrange a consultation.

When to seek urgent help

After any filler injection, or after hyaluronidase, contact the clinic immediately if you notice pain that is strong or keeps getting worse, hot, red and swelling skin with fever or pus, or hives and itching. With whitening, mottled or dark skin, any change in vision, difficulty breathing or swallowing, or swelling of the face or throat, call 998 or go to the nearest emergency department without waiting for the clinic to open. The clinic is not open around the clock, so go to an emergency department when it is closed.

Frequently asked questions

Does lip filler dissolve?

Hyaluronic acid filler breaks down by itself over months, and a doctor can speed this up with hyaluronidase. Fillers made of other materials do not respond to the enzyme. Your doctor identifies the product first.

Can lip filler be removed?

If it is HA filler, a doctor can inject hyaluronidase to reduce or remove it. The aim may be partial correction or full removal, and more than one session is sometimes needed. Other filler types need a different approach.

Can lip fillers be reversed?

HA fillers can be dissolved, unlike some other filler types. Reversal is not exact: some of your own HA dissolves too, and the amount that clears varies from person to person.

How long does it take to dissolve lip filler?

The clinic’s treatment page says softening usually appears within 24 to 48 hours and the final result is judged at about two weeks. Laboratory work cited in a 2025 review shows that speed depends on how tightly the gel is cross-linked.

Does dissolving lip filler hurt?

Pain tolerance differs between people. The treatment page describes numbing cream beforehand and mild stinging as the enzyme goes in. Swelling and tenderness can follow for a few days.

Can I have filler again after dissolving it?

Often yes, once the area has settled. The treatment page mentions a gap of about a couple of weeks as common. The doctor confirms the timing at review and decides whether new filler suits you.
