0.5 ml or 1 ml of Lip Filler: How the Amount Is Decided

The short answer

There is no correct amount that fits everyone. A doctor chooses between 0.5 ml and 1 ml, or more, by looking at your starting lip size, the balance between upper and lower lip, the look you want and how your tissue responds. Swelling hides the true shape for the first days, so the clinic’s treatment page describes placing a measured amount, waiting two weeks and adding more only if the shape asks for it. The page says a first visit usually starts with one 1 ml syringe and that a border correction often takes half.

What 0.5 ml and 1 ml mean in practice

A millilitre is a small volume. The syringe the clinic’s page refers to holds 1 ml of hyaluronic acid (HA) filler, and the doctor shares it between the upper and lower lip, or puts most of it where the shape needs it. The lip filler treatment at the clinic page explains that a first treatment usually starts with one syringe, that a border correction or a small asymmetry often takes half, and that the doctor sets the amount at the consultation, not before it.

The two numbers therefore describe two different jobs more than two sizes of the same job. On that page, half a millilitre goes with border definition and small asymmetries, and one millilitre with a first treatment that adds volume. The page also notes that splitting the work across two visits holds a better shape than one large volume in a single sitting.

What studies report about volumes

Published studies show a wide spread, and none of them sets a target for you. The table lists what three sources report, with the conditions under which each number was produced.

Source Who was treated Volume reported
FDA summary for Restylane Kysse, pivotal trial 185 adults treated with Restylane Kysse (of 273 in the trial); over 90% had very thin or thin lips at baseline Median 1.75 ml in the lips for the first treatment and touch-up combined (range 0.60 to 3.90 ml)
Same trial, retreatment at week 48 Participants who chose retreatment Median 1.00 ml (range 0.10 to 2.80 ml)
Menezes et al., 2025 253 adult women at five centers in Brazil Mean 1.06 ml (standard deviation 0.21 ml) of one HA filler in the lips and perioral zones

A third team, Kestemont and colleagues, followed 73 patients for 18 months in an open-label study in which two authors list the filler’s manufacturer as their affiliation, and reported a mean total of 0.9 ml (standard deviation 0.5 ml) over the whole 18 months, including reinjections. Studies count volume in different ways, per lip, per patient or per session, so compare their figures with care. The Kysse trial investigators were advised not to exceed 1.5 ml per lip in one session. That limit is a ceiling for the study, not a target.

What your doctor weighs when choosing the amount

The treatment page lists what the doctor assesses before selecting the product and the technique: lip anatomy, proportions, smile dynamics, skin quality, asymmetry, previous filler history and the style you want. In practice these points shape the amount.

  • Starting size. The Kysse trial graded lip fullness on a five-step scale and enrolled mainly people whose lips were graded very thin or thin. Your starting grade shapes how much change is reasonable.
  • Balance between the lips. Papers describe different reference ratios. One cites about 1:1.6 for the vertical height of the upper and lower lip in young Caucasian women, while another cites a US survey in which most respondents chose 1:1. The ratio you want is a personal and cultural choice that the doctor discusses with you.
  • Purpose. Defining the border, adding body, lifting the corners and restoring lips that have thinned with age call for different placements and different amounts.
  • Previous filler. The doctor asks what was injected before, when and where.
  • Product. The treatment page says hyaluronic acid fillers differ in texture, softness, flexibility and lifting capacity, so the doctor matches the gel to the result you want.

For the same reasons, a doctor can give the two lips different amounts. Our article on Russian lips and how they differ from classic lip filler shows how technique changes where the gel goes and how much height it adds.

Why a first visit is often a cautious one

Lips swell more than most areas. The treatment page says the true shape shows once swelling settles over about a week and that the doctor can review the lips at two weeks, with the final shape judged at two to four weeks. A result that looks generous on day one can settle into something smaller. Adding more on the spot could push the final shape beyond what you asked for.

The trial designs reflect the same idea. In the Kysse trial, a touch-up was offered at week 4 if the first treatment did not reach the correction wanted. In the Kestemont study, a touch-up was allowed at day 21, and the protocol allowed up to two planned reinjections, at 6 or 9 months and at 12 months; 56% of lips were injected only once. The authors of that study add that placing the minimum volume needed for the aesthetic result helps to avoid overfilled, unnatural shapes and reduces adverse events.

Volume also links to side effects. The FDA summary reports an exploratory comparison in which 19.8% of Kysse participants who received 2.7 ml or less in total across the lips and the area around the mouth had related adverse events, against 22.8% of those who received more. The FDA describes the difference as slight, and the trial was not designed to test volume, so read it as a small signal only. If a result ends up fuller than you wanted, an HA filler can be reduced; our article on dissolving lip filler with hyaluronidase explains when that is used.

How to prepare for the conversation

You help the doctor most by describing the result in words and pictures. Bring two or three photos of lips you find natural and say what you want to change: size, outline, symmetry or corner lift. Mention any earlier filler, cold sores, dental work planned in the coming weeks and any medicines you take, because the treatment page lists these as reasons to adjust or postpone. The same page says lip filler is not suitable during pregnancy or breastfeeding, so raise either one with the doctor at the start. Ask what amount the doctor proposes for the first visit, why, and when the review takes place. Your doctor sets the exact plan for your treatment.

If you would like a doctor to assess your lips and explain the options, you can arrange a consultation.

When to seek urgent help

After any lip filler injection, contact the clinic immediately if you notice pain that is strong or keeps increasing, or signs of infection such as spreading heat, redness, swelling, fever or pus. With whitening, mottled or dark skin, any change in vision, difficulty breathing or swallowing, or swelling of the face or throat, call 998 or go to the nearest emergency department without waiting for the clinic to open. The clinic is not open around the clock, so go to an emergency department when it is closed.

Frequently asked questions

How much lip filler do I need?

It depends on your starting lip size, the balance between the lips and the change you want. The doctor proposes an amount after examining you and usually reviews the result after about two weeks.

Is 0.5 ml of lip filler enough?

For a border correction or a small asymmetry it can be, according to the treatment page. For a visible gain in volume, a doctor may plan more, or split the work over two visits. The decision follows the exam, not a fixed rule.

Is 1 ml of lip filler a lot?

One millilitre is the amount in a single syringe and the usual starting point on the treatment page. In the FDA summary for Restylane Kysse, the median total for the lips was 1.75 ml, so 1 ml sits within the range studied.

Can I start with less and add more later?

Yes, and the treatment page describes this approach. A touch-up at a review visit lets the swelling settle first. Both trials allowed a touch-up a few weeks after the first injection.

Do the upper and lower lip get the same amount?

Not necessarily. The doctor may place more in one lip to balance the proportions or correct a difference between them. The exact split depends on your anatomy and the style you choose.

Will a top-up need the same volume as the first treatment?

Not always. In the Kysse trial, the median volume at retreatment was 1.00 ml against 1.75 ml for the first treatment and touch-up together. Your doctor decides the top-up amount after examining the lips.
