The short answer
Botox works the same way in men and women: it temporarily relaxes the muscles that crease the skin. The plan is what differs. Men typically have stronger facial muscles, so the doctor may adjust the dose and the injection points, and most treatments aim to soften lines while keeping natural movement. In the US prescribing information for Botox Cosmetic, men responded less often than women at the standard frown-line dose, which is why a review visit after treatment matters. Your doctor sets the plan for your face.
Why the plan differs for men
Botulinum toxin blocks the signal that tells a muscle to contract. A line that forms when you frown or raise your brows softens because the muscle underneath moves less. That mechanism does not depend on sex, and the steps of botulinum toxin treatment at MED YU MED are the same for men and women. The treatment page says the doctor adjusts dose and injection points to muscle strength, brow shape and the result you want.
The anatomy does differ. The introduction of a 2022 paper in Aesthetic Surgery Journal summarises the literature this way: compared with women, men typically have greater skeletal muscle mass, including the facial muscles that cosmetic injections target, along with greater facial movement and more severe lines. The same paper lists a higher density of facial blood vessels, a more pronounced supraorbital ridge and a greater glabellar projection among male facial features. Its authors add that botulinum toxin has proven somewhat less effective in men in clinical studies, and that expert consensus panels typically recommend higher doses for male patients.
That is the reasoning behind individual dosing. A dose that relaxes one person’s frown muscles fully may leave a stronger muscle partly active.
What the trial data show
Men make up a small share of the evidence. A 2020 systematic review in Dermatologic Surgery counted 19 randomized trials of botulinum toxin A for facial lines and found that men made up 13.9% of participants.
The Botox Cosmetic label gives one direct comparison. In the two pooled frown-line studies, where treated participants received 20 units, most subjects were women (82%). An exploratory analysis by gender reported that at day 30 the investigators rated 85% of 334 women as responders, against 59% of 71 men. By the participants’ own ratings, 93% of women and 72% of men were responders. In the investigator rating, a responder had no or mild lines at maximum frown. The label calls these analyses exploratory, so treat the numbers as a signal and not a forecast for any one person.
A 2005 randomized dose-ranging trial in 80 men compared four doses in the frown area. The higher doses gave a longer effect and a higher peak response than the lowest one, and side effects did not rise with dose. This describes one group in one trial, not a dose for you: the label lists one frown-line dose for all adults, and your doctor chooses the dose after examining your muscles.
How the doctor sets the plan
The consultation starts with movement. The doctor watches you frown, raise your brows and squint, and compares the lines you see at rest with the lines that appear in motion. Muscle strength, brow shape and your own goals then decide which points to treat and how much product each one gets.
The US label approves Botox Cosmetic in adults for moderate to severe frown lines, crow’s feet, forehead lines and platysma bands in the neck. In a survey of aesthetically oriented men quoted in the 2022 paper, 60% of male respondents chose glabellar lines as the area they would most likely treat first, and the two top motivations were looking good for their age and looking more youthful.
The clinic’s treatment page says the goal is a natural look rather than a smooth one. That means leaving some movement in the forehead and around the eyes. The page also notes that temporary heaviness of the brow, a drooping eyelid or slight asymmetry can occur and passes as the product wears off.
Age comes up often, and the label sets no ceiling or starting point beyond adulthood. It notes that Botox Cosmetic is not recommended under 18. In the frown-line studies the mean age was 46, and responder rates appeared higher in people under 65 than in people 65 and older. Only 23 treated participants were 65 or older, so that comparison rests on a small group. Whether to start now or wait depends on the lines you have, and your doctor advises on that.

What results look like and how long they last
The label says the injected muscles typically begin to weaken one to two days after injection, with the effect building over the first week. The treatment page describes the full effect as settling over about two weeks, so the doctor judges the result at that point and not on the day.
The label puts the duration of effect for frown lines at approximately three to four months. The 2005 trial in men found that duration rose with dose, which is one more reason the dose is tailored. The Medication Guide says it is not known whether Botox Cosmetic is safe and effective when used more often than once every three months, and the treatment page follows that limit.
Botox acts on lines that muscle movement creates. Lines that stay visible when the face is completely still, or hollows and skin texture, respond to other treatments. Botox vs Dermal Fillers: What Each One Actually Does explains where that boundary sits.
What to tell your doctor beforehand
The Medication Guide for Botox Cosmetic asks you to report several things before treatment:
- a disease that affects muscles and nerves, such as ALS, myasthenia gravis or Lambert-Eaton syndrome;
- allergies to any botulinum toxin product, or a past reaction to one;
- breathing or swallowing problems, and bleeding problems;
- planned surgery, or surgery on your face;
- weak forehead muscles, drooping eyelids or any other change in how your face normally looks;
- every medicine, vitamin and supplement you take, and which botulinum toxin product you received before.
The treatment page adds that blood thinners need the doctor’s approval first. Treatment does not go ahead over an active skin infection at the injection site.
When to seek urgent help
Call 998 or go to the nearest emergency department if you have trouble swallowing, speaking or breathing, a sudden change in vision, or skin near an injection that turns white, mottled or dark. The US label says swallowing, speech and breathing problems can appear hours, days or weeks after an injection. Tell the emergency team that you had botulinum toxin injections and when. Contact the clinic at once if you notice severe or worsening pain, signs of infection, a drooping eyelid, double vision, marked asymmetry or muscle weakness beyond the treated area. The clinic does not operate around the clock, so do not wait for it to reopen if a symptom is severe or getting worse.
Frequently asked questions
Is Botox for men?
Yes. The US label for Botox Cosmetic covers adult patients, and men are a recognised group among them. The difference lies in the plan: stronger muscles and different brow shape change the dose and injection points. A doctor examines you first and explains what suits your face.
Do men need more Botox than women?
Possibly. Published reviews link male anatomy, especially greater facial muscle mass, with a need for higher doses, and a 2005 trial in men found better results at higher doses. The label lists one dose table for everyone, and your doctor decides the amount after assessing your muscles.
Do Botox results look different in men?
The effect on the muscle is the same. In the label’s frown-line studies, fewer men than women met the responder definition at day 30, which is one reason the doctor reviews the result at about two weeks. Many men prefer keeping some movement, and the plan can reflect that.
What age do men usually start Botox?
There is no set age. The label covers adults, and it does not recommend use under 18. The timing depends on the lines you have at rest and in motion and on what bothers you. Your doctor advises whether treatment makes sense now or whether it can wait.
How long do Botox results last in men?
The label gives approximately three to four months for frown lines, and this figure comes from trials that mostly enrolled women. The 2005 men-only trial found that duration rose with dose. Your own result depends on your muscles and the dose, and your doctor sets the schedule for your treatment.
Will Botox make a man look frozen?
A careful plan aims to keep natural expression, but results vary from person to person. The clinic’s treatment page says the aim is controlled muscle relaxation with natural expression kept. Heaviness of the brow or an eyelid that droops slightly can occur and passes as the effect wears off. Tell the doctor early if you want full movement.
